CORPORATE MEMBERSHIP AGREEMENT

This Corporate Membership Agreement (the "Agreement") is entered into between EMS Spa ("Company") and the Corporate Member.

1. MEMBERSHIP TERM
The initial term of this Agreement is 24 months, commencing on the date of signing.

2. MEMBERSHIP FEES
Corporate Member agrees to pay the corporate rate as specified in the membership details.

3. CORPORATE BENEFITS
Corporate Members receive group discounts, dedicated account management, and corporate wellness programs.

4. CANCELLATION POLICY
Corporate Member may cancel this Agreement with 60 days written notice after the initial term.

5. EMPLOYEE ELIGIBILITY
Corporate Member must maintain a minimum of 10 active employee memberships.

6. RULES AND REGULATIONS
All employees must abide by all Company rules and regulations, which may be amended from time to time.

7. LIABILITY WAIVER
Corporate Member acknowledges that use of the facilities involves risk and waives any claims for injury.

8. GOVERNING LAW
This Agreement shall be governed by the laws of the state in which the Company operates.
